    We collect information necessary to provide specialized parent counselling and training. This includes: Personal Information: Name, date of birth, contact details (phone, email, address), and emergency contact information.
  • Sensitive Health Information: NDIS participant details (for your child), family history, developmental profiles, and clinical notes from our sessions.
  • Financial Information: Billing details and NDIS plan management information.

3. How We Collect Information
We collect information directly from you via our Intake FormService Agreement, and during our Telehealth sessions. We may only collect information from third parties (e.g., your child’s OT or GP) with your express written consent.

4. Purpose of Collection and Use
We use your information for the primary purpose of providing parent counselling and capacity-building services. This includes:

  • Developing and implementing your Support & Wellness Plan.
  • Communicating with you regarding appointments and care coordination.
  • Billing and NDIS compliance reporting.

5. Disclosure of Information
Your information remains confidential and will not be shared without your consent, except where:

  • Mandatory Reporting: There is a serious risk of harm to yourself, a child, or others.
  • Legal Obligation: Information is subpoenaed by a court of law.
  • NDIS Compliance: Required for audits or plan reviews as per your Service Agreement.

6. Data Security and Storage
We take reasonable steps to protect your data from misuse, loss, or unauthorised access.

  • Digital Storage: Client records are stored in a secure, encrypted practice management system.
  • Retention: Records are kept for the legally required period and destroyed securely when no longer needed.

7. Access and Correction
You have the right to request access to the personal information we hold about you and to ask for corrections if you believe the information is inaccurate.

8. Notifiable Data Breaches
In the event of a data breach likely to result in serious harm, we will notify you and the Office of the Australian Information Commissioner (OAIC) as soon as practicable, in accordance with the Notifiable Data Breaches (NDB) scheme.
